In April 2024, the Centers for Medicare & Medicaid Services (CMS) finalized its Minimum Staffing Standards for Long-Term Care Facilities, setting a new benchmark for quality and safety in nursing homes.
While these standards aim to improve resident outcomes, they also pose operational challenges, particularly for facilities already struggling with staffing shortages and rising costs.
In this post, we break down what the CMS rule entails, how it will be implemented, the impact on your operations, and how technology like our last-mile SaaS delivery platform can ease the transition.
The final rule outlines three key requirements:
Total Nursing Staff: Each facility must provide at least 3.48 hours per resident per day (HPRD) of total nursing care.
24/7 RN Coverage: A Registered Nurse must be on-site at all times, seven days a week.
Ongoing Facility Assessments: Facilities must conduct regular assessments to ensure staffing aligns with residents’ specific needs.
The CMS rule will be rolled out in phases, with different timelines for urban and rural facilities:
By May 11, 2026:
Meet the 3.48 HPRD requirement
Provide 24/7 RN coverage
By May 11, 2027:
Fulfill individual minimums of
0.55 HPRD for RNs
2.45 HPRD for Nurse Aides
By May 10, 2027:
Meet the 3.48 HPRD requirement
Provide 24/7 RN coverage
By May 10, 2029:
Fulfill individual minimums of
0.55 HPRD for RNs
2.45 HPRD for Nurse Aides
Facilities may apply for temporary hardship exemptions if they can demonstrate factors like workforce unavailability.
While well-intentioned, the mandate introduces several operational and logistical concerns:
Staffing Shortages: Finding and retaining qualified nurses and aides is already difficult.
Rising Labor Costs: Meeting the requirements may require significant investment in human resources.
Increased Administrative Load: Regular assessments, tracking, and reporting demand time and accuracy.
Logistical Distractions: Tasks like medication deliveries or transporting lab samples can divert clinical staff from direct resident care.
Our last-mile delivery platform is purpose-built to support senior care operations and reduce non-clinical workload. Here's how we help:
Easily manage and monitor all your deliveries—prescriptions, lab samples, medical records, supplies—through one dashboard.
We analyze traffic patterns, delivery needs, and costs to suggest the most efficient courier solutions tailored to your facility.
By outsourcing non-clinical transport tasks, you free up valuable staff time to focus on what matters most, resident care.
Automatically generate time-stamped delivery logs and chain-of-custody records to support audits and CMS compliance.
The CMS minimum staffing rule marks a turning point in long-term care, setting a higher standard for resident well-being but also increasing pressure on facility operations.
By embracing smart technology solutions like our last-mile SaaS platform, facilities can meet new requirements while maintaining exceptional care quality.
